2003年7月19日，D煤矿发生透水事故，死亡19人，2人下落不明。矿难发生后，该矿矿主隐瞒真相，没有向安全检查监督部门报告，而A县矿山煤炭管理局在得到群众举报后，害怕承担责任，也没有向上级安全生产监督管理部门报告，造成该透水事故发生七天后才成立事故抢险小组，延误了抢险时间。
